ABOUT ME
Born in the San Francisco Bay Area, I am a proud “transplant” to another Bay Area, having resided in Houston, TX since 2021. My approach to therapy is warm, flexible, and deeply informed by my background in community mental health, as well as some of my core values.

CONSUMER PROTECTIONS AND RIGHTS:
In compliance with Texas House Bill 4224 (89th Regular Session) and Section 181.105 of the Health and Safety Code, I am providing the following instructions for consumers:
1) You have a right to medical records produced in treatment with me. To request a copy of your records, you may email me at carmen@carmensimonpsychotherapypllc.com. I may bill for for time spent and/or copying fees.
2) You have the right to contact the Texas Behavioral Health Executive Council (BHEC). To contact the Council regarding a licensee, please visit their contact page:

3) You have the right to file a complaint regarding consumer protection issues. To file a complaint with the Office of the Attorney General, please visit:

4) You have a right to a “good faith” estimate of non-emergency services. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.
